Before submitting your intake form, please read the following information and disclosures. They are intended to help you understand the educational nature of TPT Pathways before requesting an educational consultation.
Submitting an intake form does not create an agreement between you and TPT Pathways, does not initiate educational services, and does not obligate either you or TPT Pathways to move forward.
What TPT Pathways Is
TPT Pathways provides educational resources, general scientific information, and operational guidance related to topics discussed in published educational and scientific literature.
If you choose to participate in TPT Pathways, educational services may include educational consultations, educational discussions, and written resources such as the TPT Playbook, Mixing Guide, Mixing Instructions, Administration Guide, and Administration Schedule. These materials are intended to help explain general concepts and techniques commonly described in educational and scientific literature.
What TPT Pathways Is Not
TPT Pathways is not a medical practice, healthcare provider, pharmacy, laboratory, or prescription service.
TPT Pathways does not provide medical care, diagnose medical conditions, prescribe medications, recommend treatment, interpret laboratory results, or establish a physician-patient or other healthcare provider relationship.

No Obligation Is Created
Submitting this intake form does not:
- Create a contract.
- Create a client relationship.
- Create a healthcare provider relationship.
- Initiate educational services.
- Guarantee that an educational consultation will be offered.
- Require either you or TPT Pathways to move forward.
- Create any obligation to purchase educational services.
If both parties decide to move forward following an educational consultation, a separate TPT Pathways Participation Agreement will be provided before payment is accepted and before educational services begin.
